LIABILITY RELEASE FORM
PLEASE READ THIS UNCONDITIONAL LIABILITY
RELEASE FORM AND ACTIVITY RULES AS THEY HAVE
LEGAL & BINDING CONSEQUENCES ON YOUR RIGHTS AND
IMPACT YOUR PARTICIPATION IN THE LETOURDECHRIST (LTDC)
CYCLING CLUB’S YEARLY ACTIVITIES. ONCE YOU READ & UNDERSTAND
THE RULES AND LIABILITY RELEASE, INDICATE YOUR AGREEMENT & SIGN.
ACTIVITY RULES
1) Cyclists under age 18 must be accompanied by an adult.
2) Cyclists are expected to wear a helmet and to know and obey the rules
of the road.
3) Cyclists should ride no more than two abreast.
4) Cyclists must review the Florida Bicycle Law Enforcement Guide prior
to the activities.
5) Runners and Walkers under age 18 must be accompanied by an adult.
6) Runners and Walkers must run or walk on the sidewalk and obey the
rules of the road.
7) Children under age 13, may ride their bicycles, tricycles, scooters, or
skates, on the sidewalk, accompanied by an adult Runner or Walker;
and are expected to wear a helmet.
LIABILITY RELEASE FORM
In signing this absolute and unqualified release from all liability, I acknowledge
that I understand its intent, and for myself, my heirs, executors, administrators
and representatives, do hereby agree and WAIVE, RELEASE, DISCHARGE, HOLD
ARMLESS AND PROMISE TO INDEMNIFY AND NOT TO SUE Alejandro E.
del Real, Le Tour de Christ (LTDC) Cycling Club, any organization or parish of the
Catholic Church or other Churches or Congregations in the Tours, any athlete
participating in the activity, any corporate or personal sponsors, any volunteer,
any cooperating organizations and any other parties connected with these activities
in any way together with their respective successors and assigns (the Sponsors),
singly and collectively, from and against any blame and liability for any injury,
harm, loss, inconvenience, or any other damage of any kind whatsoever, to include
Sponsors own negligence, which may result from or be connected in any way to my
participation in the Le Tour de Christ (LTDC) Cycling Club’s Activities (Club Owned
and/or Third Party) throughout the year. I hereby represent that: (1) I am physically
capable of participating in these activities; (2) that my bicycle and any other
equipment I may use are in good working condition; (3) the safekeeping and security
of personal property is my responsibility; (4) I will observe all applicable traffic and
activity rules, and (5) I will wear a helmet and conduct myself in a safe and prudent
manner while participating. I acknowledge my voluntary participation, the inherent
danger in these activities and assume the risk of such participation.
I further acknowledge having been provided a copy of the Activity Rules, Tour Safety,
Code of Conduct, Acknowledgment by Athlete, Acknowledgment of Children,
afforded ample opportunity to read them and have any questions about them
answered to my satisfaction. I hereby agree to abide by the Activity Rules, Tour Safety,
Code of Conduct, Acknowledgment by Athlete, Acknowledgment of Children, as a
precondition for my participation. I hereby consent to and permit emergency
treatment in the event of injury or illness while participating in these activities.
At various times members of the Le Tour de Christ (LTDC) Cycling Club videotapes
and photographs these activities to be submitted to the club's website and/or the local
media. By participating in any of the yearly activities from LeTourdeChrist (LTDC)
Cycling Club, I/we hereby authorize Le Tour de Christ (LTDC) Cycling Club and it’s
administrators, to reproduce, copy, exhibit, publish, broadcast or distribute any and
all such tapes or photographs.
I hereby WAIVE, RELEASE, DISCHARGE, HOLD HARMLESS AND PROMISE TO
INDEMNIFY AND NOT TO SUE the Sponsors from any damage I may sustain
because of any breach of these representations/ acknowledgments or violation of
these activities’ rules.
